AGENDA

Consider:

• Approval of claims, monthly reports and purchase orders.

• Rollover service contracts between the Muskogee County Health Department and Ban A Pest, AT&T Mobility, Maternal & Family Practice, and American Document Shredding.

• Confirmation of service order between the Muskogee County Health Department and AT&T.

• Claim 324A form from Eastern Oklahoma Circuit Engineering District 2.

• Bridge inspection invoice from Eastern Oklahoma Circuit Engineering District 2.

• Bridge owner report of conference form from Eastern Oklahoma Circuit Engineering District 2.

• Consultant invoice from Eastern Oklahoma Circuit Engineering District 2.

• Muskogee functional class revision application and information from the Oklahoma Department Of Transportation.

• Memorandum of Understanding with Muscogee (Creek) Nation for Agency Hill and Fern Mountain Road projects.

• Detention services agreement with Sequoyah Enterprises Inc.

• Application and utility permits for the following locations: 1 mile west of the U.S. 69 and Fern Mountain Road intersection; about 2.4 miles east of U.S. 69 and Harris Road intersection; 1.5 miles west of U.S. 69 and Fern Mountain Road intersection; 2.5 miles west then 1 mile north on North 64th Street from U.S. 69 and U.S. 64 intersection; east of North 24th Street and Harris Road, parallel to Harris Road, aerial for 1,400 feet, bury along Harris Road for 1,500 feet.

• Muskogee City-County Port Authority Board of Directors to award a construction contract bid under Community Development Block Grant contract 15449 ED12 in accordance with the Department of Housing & Urban Development and Oklahoma Department of Commerce guidelines and the Oklahoma Competitive Bidding Act on behalf of Muskogee County as the grantee.

• Muskogee City-County Port Authority Board of Directors to administer and oversee the execution and completion of a construction contract awarded under Community Development Block Grant contract 15449 ED12 in accordance with Department of Housing & Urban Development and Oklahoma Department of Commerce guidelines and the Oklahoma Competitive Bidding Act and in coordination with grantee and appropriate Muskogee County officials.

• Resolution authorizing disposition of equipment, a Kenmore dehumidifier, for Muskogee County Clerk’s Office.

• Replacement of Bridge #38/NBI02299.

• Resolution to accept six-month bids.

• Funds commitment letter for hazard mitigation program.

• Bids for elevator maintenance.
